Welcome to Hovis Orthopaedic Clinic and thank you for choosing us to serve your medical needs.

Our two conveniently located clinics offer comprehensive care for injuries and illnesses of the knee, shoulder and other joints, arthroscopic and reconstructive surgery of the knee and shoulder, joint replacement surgery, and sports medicine. The care we provide encompasses acute care for injuries such as fractures, joint injuries, sports medicine, and work related injuries.

We commit to offering quick response (same day appointments if needed) and to deliver an exceptional level of care coupled with compassion and concern.

Hovis Orthopaedic Clinic assists the primary care physician with close coordination and always serves as a member of the patient care team to provide clinical assessment, diagnosis and treatment.

APPOINTMENTS
For initial appointments, patients should be at the clinic 15 minutes prior to the scheduled appointment time in order to complete necessary paperwork. For follow-up appointments, patients should arrive 10 minutes prior to the scheduled appointment time. For physical therapy appointments, patients should arrive at the time of their scheduled appointment.

OFFICE HOURS
Monday through Friday, 8:00 a.m. to 5:00 p.m.

CANCELLATIONS
If you are unable to keep your appointment for any reason, please give a 24-hour advance notice so we can schedule someone else or reduce the waiting time for other patients.

REFERRALS/AUTHORIZATIONS/PROOF OF INSURANCE
If your insurance company requires a referral from the primary care provider, you must provide the clinic with the appropriate referral paperwork at the time of the initial appointment. A patient can only be seen for what has been authorized by the primary care provider or approved by the appropriate referral agency.

Proof of insurance (card or contract) must be provided at the initial visit. A copy of the patient's current insurance card will be maintained on file to ensure prompt and accurate filing. All patients will be asked to complete new paperwork every six months to keep our records current.

For Second Opinions and Independent Medical Examinations the patient is required to have all medical records, radiology and other diagnostic studies at the clinic prior to the appointment.

FINANCIAL POLICY
Insurance co-pays and deductibles are due when the services are rendered. Co-pays cannot be waived. Patients without insurance are expected to pay at the time of service unless special arrangements are made prior to the appointment. Payments for second opinions and independent medical examinations are due in advance of services rendered.

MEDICAL RECORDS
All medical records and x-rays are the property of Hovis Orthopaedic Clinic. Records can be transferred to another physician or medical facility with the completion of appropriate authorization forms. A 24-hour advance notice must be given if the patient plans to pick up the records.

PRESCRIPTIONS AND REFILLS
Patients of Dr.William Hovis and Dr.David Hovis, the following policy will be strictly adhered to:

Due to the ongoing problem with fraudulent prescriptions, the doctors will no longer be calling in pain medicine after hours or on the weekends. All requests should be called in before 2:00pm Monday through Thursday and 12:00 pm on Friday. Any request called in later will be taken care of the next business day. No Exceptions!
